Photographs of SMN members may be used in various SMN communications including the website. Group photographs taken at SMN events may be used without identifying individual members. For individual photographs, please indicate your permission for use

Please complete this form if you are a new member or submitting changes. Membership runs from July 1st - June 30th.

1.  Membership is open to any person who shares the corporation's mission and vision with a desire to support the program's objectives.

2. Membership dues are $25.00 and are accessed the first month of the fiscal year which is July.
If time of admission is within three months of due date of annual dues, new member shall pay dues for
admission and not required to pay upcoming annual dues.

3. Any member in good standing is eligible to serve on the Board of Directors and/or elected as an officer.

4. Membership may be terminated voluntarily by the member or involuntarily by the action of the board whenever in its judgement the best interests of the corporation will be served thereby.

5. Annual meetings of the members shall be held at such place or places, within or outside the state of Oklahoma.
A member shall be considered present at any meeting if during the meeting he/she is present via telephone.
Notice of annual meeting will be delivered in a timely manner.   Attendance is not required.
